Inpatient Rehab Center

Helping You Get Back On Your Feet

CHRISTUS Inpatient Rehabilitation Center’s goal is to help you regain the highest level of function and independence possible. Your comfort and care is our priority, and we value your input. If you have any recommendations, questions or concerns at any time during your stay, discuss these with any interdisciplinary team member. We appreciate the opportunity to meet your rehabilitation needs.

What is expected of me in inpatient rehabilitation?

During the first three days, you will meet with our interdisciplinary team of specialists. The treatment team will assess your functional abilities and design your individualized treatment plan. Your input is important in establishing this treatment plan.

Who is the interdisciplinary team?

  • Rehabilitation Physician—oversees your rehabilitation program and works with you and the team to determine the overall plan of care to meet your needs.
  • Rehabilitation Nurse—special type of nurse that addresses your medical and functional needs and helps you learn to manage your medical condition.
  • Case Manager—plays an integral role and serves as a contact between you and the interdisciplinary team, and ensures that the overall plan of care meets your individual needs. They also coordinate your followup, post-discharge care.
  • Physical Therapist—works with you to reach the highest level of function possible in mobility skills and addresses range of motion, strength, balance, coordination and endurance.
  • Occupational Therapist—works with you to reach the highest level of function possible in activities of daily living (ADLs) and will address eye-hand coordination and visual perception as needed.
  • Speech-Language Pathologist (as needed)—works on your communication skills and cognitive skills, and addresses any impairment in swallowing.

What do I bring with me?

  • comfortable clothing that you would normally wear
  • slacks/sweatpants
  • shirts/blouses
  • underwear, socks, shoes (with nonslip soles)
  • sleepwear
  • toiletries—toothbrush, toothpaste, denture cleaner, brush/comb, deodorant
  • corrective devices or prosthetic devices


Your family will be responsible for doing your laundry. Please label all of your belongings. CHRISTUS Shreveport-Bossier Health System is not responsible for the loss of any personal belongings unless they are deposited in the hospital safe.

Discharge Planning

Your discharge date is determined by your rehabilitation physician and members of the interdisciplinary team. As soon as the discharge date is set, the case manager will inform you and your family so you may arrange follow-up care, order any equipment necessary and make any home modifications as needed.

Follow-Up Telephone Call

To make sure that the rehabilitation program you received met your needs, the case manager will be contacting you within three days after discharge. Please tell us how your rehabilitation at home is going.
